Permit to Transport Cadaver: Php 65.00; Permit to Conduct Promotional Sales and Activities: Php. 65.00/day & Secretary’s Fee of 40.00; Certificate of Posting: Php. 113.00;

Schedule of Service Availability:

Monday to Friday 8:00am to 12:00 noon & 1:00pm to 5:00pm
